The need for accessible palliative care is more pressing than ever in Nepal, where significant strides are being made to improve the availability and affordability of essential medications. A recent paper titled “Availability and Affordability of Essential Palliative Care Medicine in Nepal” has been published in the renowned Journal of Pain and Symptom Management, marking a critical step toward shedding light on the current challenges and gaps in accessing palliative care in the region. This study offers valuable insights into the accessibility of these medicines and examines the economic and logistical barriers that patients often face.

Building on this momentum, a Palliative Care formulary has been drafted to establish a standardised guide for the essential medications required in palliative care practices across Nepal. This formulary is currently under expert review by seasoned palliative care specialists, who are fine-tuning the recommendations to ensure the highest standards of care and accessibility. Once approved, this formulary will provide healthcare practitioners with a unified reference for prescribing and managing palliative care medications, contributing to more consistent care for patients across different regions.

In a parallel effort, a manuscript has been submitted to the Indian Journal of Palliative Care, documenting both the achievements and persistent challenges related to opioid accessibility in Nepal. Opioids, often critical in managing severe pain for palliative patients, remain difficult to access due to regulatory, educational, and logistical issues. This manuscript highlights ongoing efforts to address these barriers, providing a balanced view of both the progress made and the hurdles that still need to be overcome. It’s a candid reflection on the complexities of opioid accessibility in Nepal and offers recommendations for future improvements, aiming to inform policy changes and healthcare practices.

These combined efforts represent a significant push toward a future where all Nepalis in need of palliative care have affordable access to essential medications. Each publication and policy review strengthens the foundation for a more comprehensive palliative care system, giving hope for a future where relief from pain and suffering is within reach for all.

Autism Spectrum Disorder (ASD) is increasingly recognized as a critical neurodevelopmental disorder affecting individuals across Nepal. With growing awareness, there is a pressing need to provide support and understanding for individuals with autism and their families. This disorder encompasses a range of developmental differences, including social challenges, communication difficulties, and unique behavioural patterns, all of which affect daily life and integration within society. However, many Nepali communities lack awareness and access to resources to support affected individuals.

To address this gap, several national and international organisations, along with local government and communities, have initiated awareness campaigns, education programs, and support services for autism. As part of these efforts, a web-based educational resource PDF, titled Autism Handbook: Web-Based Educational Resource, has been produced and is available for download. This handbook aims to provide essential information and guidance to foster an autism-friendly environment that respects the needs and rights of individuals on the autism spectrum. Comprehensive information, dedicated resources, and the active participation of all stakeholders are essential for empowering individuals with autism to access educational and social opportunities, ensuring they can lead fulfilling lives

In response to the challenges posed by long COVID, Green Pastures Hospital has taken a significant step forward by developing an innovative and cost-effective approach to respiratory rehabilitation. A recently published article, “Use of Locally Produced Novel Low-Cost 3D Printed Respiratory Muscle Strength Trainer Device (RMSTD) for Long COVID Rehabilitation,” featured in Wiley, details the design, development, and clinical success of a 3D-printed Respiratory Muscle Strength Trainer Device (RMSTD) uniquely suited for treating respiratory muscle weakness in long COVID patients.

This custom-made RMSTD device was designed and produced entirely at Green Pastures Hospital, specifically for a patient who had previously undergone intensive positive airway pressure support during a prolonged ICU stay. The low-cost, 3D-printed design reflects a groundbreaking solution for managing the complex needs of patients recovering from severe COVID-19 symptoms, where respiratory muscle strength can be significantly compromised.

One of the critical aspects highlighted in the article is the local production of the RMSTD device, a factor that not only reduces costs but also enables rapid deployment in healthcare settings where resources are limited. The device’s creation involved interdisciplinary collaboration between respiratory therapists, medical engineers, and rehabilitation specialists, all of whom recognised the need for a targeted approach to address the lingering effects of long COVID on respiratory health. The article outlines the step-by-step process of designing the device, the adjustments made to accommodate patient-specific needs, and the protocols followed to ensure both safety and efficacy.

Through this project, Green Pastures Hospital aims to contribute to the broader conversation on long COVID management, demonstrating the value of accessible, locally produced rehabilitation solutions. By documenting the device’s impact on respiratory recovery in a severe COVID-19 patient, the team hopes to inspire similar low-cost innovations that can benefit COVID-19 recovery efforts globally.

The hospital’s experience underscores the necessity for adaptive solutions in the evolving landscape of long COVID care, especially for patients whose respiratory systems are profoundly affected. The article provides a detailed look at both the challenges encountered and the successful outcomes achieved, offering valuable insights for healthcare providers seeking practical, affordable tools to support patients on their path to recovery.

By sharing their findings, the team at Green Pastures Hospital is not only highlighting an effective model of respiratory rehabilitation but also contributing meaningfully to the international dialogue on innovative, resource-sensitive responses to the ongoing challenges of long COVID.
